[Opening, 0–4s] Close-up of man's face on hospital bed. Eyes open, confused, scared. He's breathing heavily. Pale skin. Suddenly his body CONVULSES. His breathing becomes labored. Camera pulls back to show full body. He's chained to the bed (survival horror context). His skin begins to look GREY/DISCOLORED around the edges — starting at fingertips, spreading inward.

[Early Transformation, 4–12s] The discoloration SPREADS across his body like infection. Veins become visible and BLACK under the skin. His eyes begin to CLOUD OVER — losing color, becoming milky/dead. Blood starts dripping from his nose (realistic, not overdone). His fingernails DARKEN and become brittle. He tries to speak but only a RAGGED GROAN comes out. His muscles visibly tense and twitch as his body changes. The camera circles him showing progression.

[Peak Transformation, 12–22s] His face DISTORTS. Jaw becomes more pronounced. Skin TEARS in places, revealing darker tissue underneath. His eyes are now PURE MILKY WHITE/BLACK. His eyes go completely DEAD (pure black pupils). One final SCREAM (distorted, inhuman, guttural). Veins pulse visibly one last time. His entire body goes RIGID. Camera focuses on his face as the last traces of humanity drain away. The scream becomes an inhuman GROWL.

[Final Moment, 26–30s] Return to normal speed. The creature that was once a man STOPS moving. Complete stillness. His head slowly ROTATES toward camera with mechanical, inhuman movement. His milky dead eyes stare directly forward. His mouth hangs open revealing blackened teeth and necrotic tissue inside. He SNARLS silently, saliva dripping. One final GROWL/SHRIEK. Camera holds on his completely transformed zombie face no humanity left, pure undead creature. Freeze on his horrifying visage.

[Audio] Realistic hospital ambience at start (beeping monitors, fluorescent hum). Man's labored breathing becoming RAGGED and GUTTURAL.
